KDE applications automatically sets large icon because KDE itself contains code that find large icon and attaches it to X-Window. Non-KDE applications set usually small icons (like Skype and YopuMagic). It is allowed that any program may change X-window icon at any time. For example, Gimp sets a small thumbnail of image that is opened by user. Gnome Shell implementation of Alt+Tab tries to find desktop file and load large icon from theme, but I don't sure that Gimp thumbnails is working in Gnome.
